Getting Ready For Your First Visit

In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

  • Insurance information and card
  • Photo ID
  • Referral form, if applicable
  • New Patients: please download the patient registration
  • A list of your current medications
  • Medical records from prior visits that you think may be helpful
  • Any pertinent x-rays

We try to simplify the preparation of your insurance claims; thereby holding down costs that are unrelated to the delivery of quality medical care to you and your family. We file our insurance claims electronically.

Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some insurance companies pay a fixed allowance for certain procedures, and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, including co-insurance, or any other balance not paid by your insurance company.

Financial Policy

Your clear understanding of our financial policy is important to our professional relationship. The following is a statement of our financial policy that we require you to read prior to any treatment. Please contact us if you have any questions regarding our fees.

  • All patients must complete our patient information form before seeing the physicians.
  • Payment is due at the time of service.
  • We accept cash, checks, Visa, MasterCard, Discover, or American Express.

Emergencies

Our Office offers 24-hour physician coverage. When an emergency arises after office hours, call 911 or (504) 208-2685. You will then be instructed as to how to reach our on call provider who will then respond to your call. In this day of complex insurance programs, it is absolutely necessary that you follow your insurance policy regarding emergencies or you may be financially responsible. We therefore recommend that you review your coverage frequently so that you know what your contract requires in an emergency.
